Abstract
In 1989, a modern Russian icebreaker came within minutes of meltdown. Massive radiation poisoning was narrowly averted, saving Murmansk, a city of 440,000 people, from nuclear disaster. How the six-year-old nuclear-powered Rossiya came to almost destroy a city the size of Wellington or Christchurch is worrying. Simply put, it was human error
